Thank you for choosing Valley Rehabilitation of Sun City West. Please complete these forms and bring them with you. If you’re unable to fill these out before your appointment, please arrive 30 minutes prior to your appointment so that we can collect the appropriate information prior to meeting with a physical therapist.
MEDICARE PATIENTS
In addition to the paperwork we are required by CMS to have our patients complete a health status questionnaire. This is done using a digital tablet device at your evaluation, mid-treatment, and discharge. Please arrive 10 minutes prior to your appointment to complete this digital questionnaire.

Motor Vehicle and Liability Accident Forms
If you have been injured in a motor vehicle accident or in a situation that involves personal injury, Valley Rehabilitation of Sun City West can help! At Valley Rehabilitation, we will assist you through the process of filing a lien and will communicate with your attorney if necessary.

This is not a guarantee of coverage, this is made when claims are sent to your insurance carrier and claims are processed. It is the patient’s responsibility to know/understand their insurance coverage.
